REGISTRATION PROCEDURES


DROPPING A CHILD FROM SCHOOL
1. IF a child is absent for three consecutive days without notifying the classroom teacher, the teacher then;
(a)
conducts a home visit;
(b)
have the parents or legal guardian sign the “Home Visit” form;
(c)
fills out the drop slip.

The teacher then submits to the Team Area Manager the child’s file, a "drop slip" signed by the parent(s) and the school’s record book (must show number of days child has been absent without an excuse).

2. The District Team Manager
(a)
reviews the child’s file for verification of attendance and parents/family contact;
(b)
marks the record book, indicating the date, and initials it; places both sets of the child’s files (1 from the center and 1 from the office) in the “INACTIVE FILES”.
(c)
informs the Program Registrar of changes and hands in the “drop slip”.

3. The Registrar then makes changes on the Master List.
4. The Mentor Teacher returns the attendance book to the classroom teacher.

 

Note: If the child’s parents or legal guardian are not available during the “home visit”, a relative or neighbor can be informed as an indication that effort has been made to contact the family. All actions are effective immediately after the Program Area Manager signs the “drop slip” and changes have been made on the Master List. The transfer process will take three days to allow for necessary paper work and to notify the School Lunch Program.

CHILD TRANSFERS
(a)

Parents must request the transfer of his/her child 3 days in advance to the classroom teacher. In this request, parents will get a transfer form/slip from the child’s teacher;
(b)
Parents will then need to fill out the transfer form/slip;
(c)

The classroom teacher will initial the transfer slip, send the transfer slip and the child’s files to the main office, giving it to the Program Registrar;
(d)

The Program Registrar will forward the files to the Program Area Manager of the Center the child is being transferred to;
(e)

This Program Area Manager will then determine if there is space available. If available, an entrance slip indicating 3 days before actual classroom entrance is issued.
